TO ORDER BY CHECK OR CREDIT CARD:

1. Print the order form and mark down your order. Don't forget to PRINT your name and address in the space below, and sign on the signature line. 

2. Check orders need to be mailed to us; credit card orders can be mailed or (preferably) faxed. If you're using a credit card, write down your credit card information in the space provided. Mail completed order forms with check or credit card information to:  SXM Merchandise, c/o JMB, P. O. Box 1812, Plymouth, MA 02362-1812. If you're paying by credit card, you can mail or fax the form to us; we recommend fax. Fax number: 1-508-746-7898. (This number is only for merchandise orders.)

TO ORDER THROUGH PAYPAL:

Note: Items near the top of the page which say "Add To Cart" Can be Ordered online via PayPal. Or, if you prefer, you can order any items on this page this way:

1. Print the order form and mark down your order.

2. Go to https://www.paypal.com  . 

3. Using the PayPal service, send the $ for what you want to susan@jmbcommunications.com. Include in your "Message" area your name, address, email address, and the Item number(s) and quantities you want. Item numbers are printed in BOLDFACE TYPE on the order form above. If for any reason you can't get the required info in the "Message Area," then after completing the PayPal payment transaction, email the order information to us ALONG WITH YOUR RECEIPT # FROM PAYPAL; orders without receipt numbers can't be processed. Send the email to susan@jmbcommunications.com.  Never send any credit card number by email.
